Xiaoyao Powder is one of the most commonly used prescriptions in Traditional Chinese Medicine. It is a conciliatory agent that has the effects of harmonizing the liver and spleen, relieving liver depression, nourishing blood and strengthening the spleen. It is mainly used to treat liver depression, blood deficiency and spleen weakness. Pain in the flanks, headache, dizziness, dry mouth and throat, fatigue and poor appetite, or irregular menstruation, breast pain, and a stringy and weak pulse. It is commonly used clinically to treat chronic hepatitis, cirrhosis, cholelithiasis, gastric and duodenal ulcers, chronic gastritis, gastrointestinal neurosis, premenstrual tension disorder, lobular hyperplasia of the breast, etc., which are caused by blood deficiency and weak spleen.

This ancient recipe can cure cirrhosis of the liver, hypertension, breast lumps, dysmenorrhea, and cholecystitis. Taiping Huimin Hejijufang: "It is used to treat blood deficiency, fatigue, fever in the five hearts, pain in the limbs, dizziness, palpitations, red cheeks, dry mouth and throat, fever, night sweats, loss of appetite and love of lying down, as well as blood and heat fighting, irregular menstruation, umbilical and abdominal distension and pain, cold and heat like malaria. It is also used to treat weak blood and yin deficiency in virgins, disharmony between the blood and the body's defense system, phlegm, cough, hot flashes, emaciation, and gradual bone steaming."

Clinical application of addition and subtraction: For dysmenorrhea caused by liver qi stagnation, qi stagnation and blood stasis, Xiaoyao Powder can be added with Salvia miltiorrhiza, Cyperus rotundus and Leonurus japonicus to promote qi and blood circulation, regulate menstruation and relieve pain. For menstrual disorders caused by liver qi stagnation, Xiaoyaosan plus raw rehmannia (Hei Xiaoyaosan) can be used, or green peel, angelica, Taiwu, etc. can be added to promote qi and regulate menstruation. For breast sores caused by liver qi stagnation, Xiaoyao Powder with litchi seeds, tangerine seeds, and yellow peel seeds can be used to promote qi and relieve stagnation. For women suffering from menstrual headaches, Xiaoyao Powder with coix seed, Artemisia capillaris, hawthorn, etc. can be used to dispel dampness and relieve depression. For patients with chronic cholecystitis of the liver depression and spleen deficiency type, Xiaoyao Powder with Chuanxiong and Angelica dahurica can be used to dispel wind and relieve pain, or chrysanthemum, Uncaria rhynchophylla, Tribulus terrestris can be added to calm the liver and stop wind. For patients with non-icteric hepatitis of the liver depression and spleen deficiency type, Xiaoyao Powder with Cyperus rotundus, Curcuma aromatica, and Artemisia capillaris can be used to soothe the liver and promote bile secretion, or with Cortex Moutan and Gardenia jasminoides to cool the liver and promote bile secretion. For early-stage liver cirrhosis, Xiaoyao Powder with turtle shell, salvia miltiorrhiza, Curcuma zedoaria, and Trillium can be used to soften hard masses and dissolve blood stasis. For women with early hypertension caused by liver depression transforming into fire and hyperactivity of liver yang, Xiaoyaosan with moutan bark and gardenia (Danzhi Xiaoyaosan) can be used to cool the liver, reduce fire and lower blood pressure. The above-mentioned diseases can all be effectively treated. The following cases are given to illustrate this.

Case examples

Dysmenorrhea

Huang, female, 22 years old, unmarried.

For the past 4 months, the patient has had a bloated lower abdomen, an anxious temper, and irritability before each menstruation. The lower abdominal pain worsens during menstruation. The menstrual blood is purple and contains clots. The menstruation is not smooth and the amount is not much. The tongue is pale, the tongue coating is thin, and the pulse is stringy.

This is due to liver qi stagnation, qi stagnation and blood stasis. The treatment is to soothe the liver and relieve depression, nourish the blood and promote blood circulation. The prescription is Xiaoyao Powder with added ingredients.

Prescription: Bupleurum 10g, Angelica 10g, White Peony 12g, Atractylodes 12g, Poria 10g, Licorice 5g, Leonurus 15g, Salvia miltiorrhiza 15g, Cyperus 12g.

After taking 3 doses in a row, the dysmenorrhea disappeared and did not occur during the next menstrual period.

Note: The main causes of dysmenorrhea are qi stagnation and blood stasis or cold stagnation and blood stasis. This case is caused by liver depression and qi stagnation. Because the liver stores blood and women are based on blood, liver qi depression and qi stagnation will lead to blood stasis. If qi and blood are not flowing smoothly, dysmenorrhea will occur. Therefore, Xiaoyao Powder is used to soothe the liver and relieve depression, nourish blood and regulate menstruation, and Danshen, Leonurus japonicus and Cyperus rotundus are added to regulate Qi and activate blood circulation. When Qi and blood are regulated, dysmenorrhea will naturally be relieved.

Breast

Zhang, female, 18 years old, unmarried.

The patient has irregular menstruation, which may come early or late, with varying amounts of menstrual flow, light red menstrual blood, bloating and discomfort in the lower abdomen before menstruation, which also affects the chest, ribs and breasts. She is irritable and easily loses her temper, has a pale yellow complexion, poor appetite, a pale red tongue, a thin white coating, and a stringy and thin pulse.

This is due to stagnation of liver qi, which results in irregular menstruation. It can be treated with Xiaoyao San with modifications.

Prescription: 10g of Bupleurum, 10g of Angelica, 12g of White Peony, 15g of Rehmannia, 10g of Atractylodes, 12g of Poria, 12g of Cyperus, 12g of Atractylodes, 12g of Wu, 12g of Sophora flavescens, 3 slices of ginger, and 5g of mint.

Take 5 doses every month, and menstruation will return to normal after taking it for 2 months.

Note: Menstrual disorders are irregular periods due to liver qi stagnation. Stagnation of qi causes abnormal blood circulation, and restlessness in the blood sea leads to disorder. Therefore, menstruation is irregular, sometimes early, sometimes delayed. The Foot Jueyin Liver Meridian runs through the breasts, flanks, and lower abdomen. When liver qi is stagnant, there will be swelling, pain, and discomfort in the breasts, flanks, and lower abdomen. When the liver is depressed, the liver qi cannot flow freely and unimpeded, so the person becomes irritable, easily angered, has a sallow complexion, and has a poor appetite. This is due to liver qi stagnation, which affects the stomach and causes spleen and stomach weakness. A stringy pulse is a sign of liver qi stagnation. Therefore, Xiaoyao Powder is used to soothe the liver and relieve depression, and is supplemented with Shengdi to nourish yin and blood, and Taiwu, Sugeng and Xiangfu to regulate qi and menstruation. When liver depression is relieved, menstruation will be regular.

Breast

Deng, female, 33 years old, married.

The patient had two bean-sized lumps next to her left breast, which were movable and soft. The pain and distension worsened with each menstrual period, and there was sometimes a shooting pain. Because patients often suspect that they have breast cancer, they become depressed and prone to temper tantrums. After pathological examination, it was confirmed to be "cystic hyperplasia of the breast". The tongue is pale red and the pulse is stringy and fine.

This is due to liver qi stagnation, with qi stagnation blocking the liver meridian. For treatment, use Xiaoyao Powder with added ingredients.

Prescription: 10g of Bupleurum, 15g of White Peony Root, 10g of Angelica Sinensis, 10g of Atractylodes Macrocephala, 15g of Poria, 5g of Licorice, 5g of Citrus Aurantifolia, 12g of Cyperus Rhizoma, 15g of Tangerine Seed, and 15g of Litchi Seed.

After taking 3 doses, breast pain was relieved, and 7 doses of the medicine were taken 7 days before each menstruation. After taking it for 3 consecutive months, the lumps became smaller and gradually disappeared.

Note: The Foot Jueyin Liver Meridian runs through the breast, and liver qi stagnation condenses into lumpy cystic growths. Treat with Xiaoyao Powder to soothe the liver and nourish the blood, add green peel, angelica root, tangerine seed, litchi seed etc. to promote qi and disperse nodules. The liver qi is unobstructed and can flow freely, so the disease can be cured.

He, female, 35 years old, married.

In recent months, the amount of leucorrhea after menstruation has increased and become thinner, and there is dizziness, poor appetite, a slightly puffy face, a pale red tongue, a thin and greasy tongue coating, and a weak and stringy pulse. Gynecological examination revealed cervicitis.

Note: The main pathogenesis of leucorrhea is spleen deficiency and dampness, which causes dampness and turbidity to flow downward and cause the belt meridian to lose its restraint. If the Liver Qi is stagnant and cannot relieve the spleen and earth, the spleen will be weak and cannot transport and transform water and dampness, which will form leucorrhea. Therefore, Bupleurum, White Peony Root and Angelica Sinensis are used to soothe the liver and nourish the blood, Poria and Atractylodes Macrocephala are used to strengthen the spleen and eliminate dampness, and Smilax glabra and Plantago seed are used to eliminate dampness and stop leukorrhea, while Cockscomb is used to clear dampness and heat and stop leukorrhea.
